Stem Health offers two physician-led assessments: Core and Horizon. Core provides a comprehensive baseline across the main areas of your health. Horizon includes everything in Core, then adds advanced imaging, continuous glucose monitoring and more detailed follow-up testing.
Both are prepared around your health history and completed over one carefully paced day. Your physician brings the findings together with you, answers your questions and turns the results into a plan saved to your Health Story. Six months later, your care team checks in to review your progress. Six months later, your care team checks in to see what has changed and how the plan is progressing.
The difference is not the quality of care surrounding you. It is the depth of the evaluation. Core establishes a broad, detailed baseline. Horizon includes everything in Core, then adds advanced imaging, continuous monitoring and a more extensive follow-up.
The Core Assessment: A comprehensive view of your health
Core brings together 12 testing stations and approximately 20 activities over five to six hours.
Your biomarker testing takes place one to two weeks before the assessment, allowing your physician to begin the day with the results already in hand. From there, Core examines your health from several perspectives:
● A comprehensive biomarker panel, including lipid and inflammation markers
● An opening consultation with your physician, along with a physical examination and vital signs
● An electrocardiogram and VO₂ max test to assess cardiovascular function and fitness
● Abdominal imaging and, where appropriate, mammography
● A DEXA scan to measure body composition and bone density
● Grip strength, biomechanics and movement testing
● Cognitive testing, retinal imaging and a hearing assessment
● A sleep assessment with at-home apnea testing
● Nutrition and mental wellbeing consultations
● A closing conversation with your physician, bringing the day’s findings into one clear picture
● A six-month follow-up to review your progress and revisit the plan
Core is not a shortened version of the assessment experience. It offers a thorough baseline across every parameter Stem examines, with each finding considered in context and explained by your physician. For someone seeking a clear, wide-ranging understanding of their health without advanced imaging, Core offers a substantial place to begin.
The Horizon Assessment: A closer look where it matters
Horizon includes everything in Core, then extends the assessment to 17 stations and approximately 25 activities over six to seven hours.
The additional testing includes:
● Whole-body MRI, examining multiple organ systems without ionizing radiation, with physician consultations before and after the scan
● Coronary CT and calcium scoring, offering a closer look at the coronary arteries and the presence of calcified plaque
● Continuous glucose monitoring, worn after the assessment to show how glucose levels respond throughout your usual meals and routines
● Additional imaging of the brain, liver, spine and major joints
● A personal activity and nutrition plan informed by the day’s findings
● Structured risk assessments covering cardiovascular, metabolic and cancer risk
● A more extensive six-month progress assessment, including repeat laboratory testing
The experience does not become more attentive with Horizon; it becomes more extensive.
Both assessments are carefully paced, with unhurried breaks and meals planned into the day. Both close with time for your physician to explain what was found, answer your questions and agree on what comes next.
Horizon asks for a little more time because it gathers more information. Whether that added depth is useful depends on your health history, concerns and what you hope to understand.
What each assessment costs

You do not need a paid Stem Health membership to book an assessment. Both can be accessed through the free, pay-as-you-go Origin Pass. Foundation and Signature members receive preferred pricing.
Choosing the right depth
The best place to begin is with what you want the assessment to help you understand.
Core may feel right if you want to establish a comprehensive baseline across your physical, metabolic and mental wellbeing. It offers a broad view without the additional imaging and monitoring included in Horizon.
Horizon may be worth considering if your history or concerns call for a closer look, or if advanced imaging and continuous glucose monitoring would add meaningful context to your care. Your physician can help determine whether those additional elements are appropriate for you.
If you begin with Core, its findings may also help inform whether further testing is worth considering later.
You do not have to make the choice in isolation.
